Home care

HOMECARE SERVICES

We know that remaining in your own home is very appealing but that at times you might benefit from some extra support.

Rembrandt can provide you with additional help allowing you to live in your familiar surroundings with confidence and ease. We offer an extensive range of either long or short-term home care services that will assist you or your loved one and give everyone peace of mind.

There are two ways to obtain Home Care support:

1. Private engagement
Engaging a home care support private provider, which involves funding the services yourself.

2. With the assistance of Government subsidy
Starting out on the subsidy Home Care process can be confusing, that’s why we have a person dedicated to guide you through the procedure of working within the appropriate government guidelines. This includes an assessment by the Aged Care Assessment Team (ACAT). Our experienced Rembrandt Coordinator is ready to assist you with the My Aged Care process and once approved, coordinate the services that you want, the way you want them.

SUPPORT SERVICES

Depending on your level of care as determined by your ACAT (Aged Care Assessment Team), there are two types of home care services and assessments:

Entry Level Packages
Our Entry-Level Home Care Services, funded by the Commonwealth Home Support Program (CHSP), provide basic support services to assist you to remain independent at home.

Home Care Packages
For those with more complex needs, Home Care Packages, subsidised by the Government, provide specialised in-home support with a wide range of services that will assist you to remain living in your own home. An Aged Care Assessment Team (ACAT) will determine the level of assistance to be provided.

COMMUNITY SERVICES

Our Community Allied Health service provides age-friendly support to help you remain living in your own home for longer.

We have a focus on reablement and restoring, retaining or improving general strength, function, balance and quality of life. Our qualified and experienced Occupational Therapists and Physiotherapists run one-on-one sessions and group classes. The Home Care and Health Centre complement this offering with massage therapy, podiatry and dietetics. These aged care services are designed to ensure clients feel welcome and at home and well supported by our friendly staff.

COMMUNITY SERVICES AVAILABLE

Social groups

Rembrandt Community Allied Health services operate from Rembrandt Court in Oaklands Park (southern area) and from our newly renovated Home Care & Health Centre in Green Fields (northern area). We provide centre-based (including transport to and from and lunch) and home services when you’re not able to get to a centre.

One-on-one therapies

Occupational Therapy (OT)
includes falls prevention, home safety and modifications, discharge planning, assistive aids prescriptions, and post stroke upper limb rehabilitation.

Physiotherapy
offering a wide range of treatment options to relieve pain, restore movement and function, and prevent further problems.

Massage Therapy
presents a smooth gentle, flowing style that promotes general relaxation, relieves muscular tension, plus improves circulation and range of movement. Your therapist will aim to relax, revive and rejuvenate you at a massage depth that does not cause discomfort.

Podiatry
covers assessment, diagnosis, treatment, management and maintenance of conditions of the feet and legs. Foot care is especially important for people with specific conditions such as diabetes, which can cause foot problems.

Group classes

When you join a group session you can also enjoy socialising with others over a free cuppa in our café afterwards.

Allied Health Exercise Classes
Our small group classes include better balance, stretch & strength, social movers & thinkers, and all hands-on-deck.
