Support services

We offer an extensive range of home care services that will assist you or your loved one to live at home.

There are two types of home care services and assessments which can provide access to either Entry Level Services or Home Care Packages.

Our Home Care Coordinator will be pleased to assist you to determine what level of care is appropriate for you and how to access this care.

“It takes an extraordinary amount of skill, patience, and kindness to work in aged care and to care for our residents with heart.”

Staff Testimonial

Our basic support services assist you to remain independent.

Our Entry-Level Home Care Services, funded by the Commonwealth Home Support Program (CHSP), provide basic support with the following:

  • Domestic, gardening and home modifications
  • Personal care
  • Social support including group classes
  • In-home respite
  • Shopping and transport
  • Food preparation and delivery

For those with more complex needs, Home Care Packages provide specialised in-home support.

If you are assessed as eligible, you will be informed which level of package you are approved for and then placed in the national queue. When you reach the top of the queue, you will be assigned a package and can then choose a provider such as Rembrandt to deliver your services.

Home Care Packages range from Level 1 (low level support) to Level 4 (high level support). Once assigned your package, a dedicated and experienced Rembrandt Home Care Coordinator will work with you to plan the services that you want, the way you want them, in the comfort of your home. Our fee schedule is available here.
