We know that remaining in your own home is very appealing but that at times you might benefit from some extra support. Rembrandt can provide you with that additional help allowing you to live with confidence and ease using either long or short-term services. We offer an extensive range of home care services that will assist you or your loved one to live at home.
There are two options to obtain Home Care – private engagement or with the assistance of a government subsidy.
Engaging a home care support private service involves funding the services yourself.
Starting out on the subsidy Home Care process can be confusing, that’s why we have a person dedicated to guide you through the procedure of working within the appropriate government guidelines. This includes an assessment by the Aged Care Assessment Team (ACAT). Our experienced Rembrandt Coordinator is ready to assist you with the My Aged Care process and once approved, coordinate the services that you want, the way you want them.

Services We Offer
Depending on your level of care as determined by your ACAT (Aged Care Assessment Team), there are two types of home care services and assessments:
- Entry Level Services or
- Home Care Packages
Entry Level Packages
Our Entry-Level Home Care Services, funded by the Commonwealth Home Support Program (CHSP), provide basic support services to assist you to remain independent at home, for example:
- Domestic, gardening & home modifications
- Personal care
- Social support including group classes
- In-home respite
- Shopping & transport
- Food preparation and delivery
Home Care Packages
For those with more complex needs, Home Care Packages provide specialised in-home support with a wide range of services that will assist you to remain living in your own home.
If you are assessed as eligible, you will be informed which level of package you are approved for and then placed in the national queue. When you reach the top of the queue, you will be assigned a package and can then choose a provider such as Rembrandt to deliver your services.
Home Care Packages range from Level 1 (low level support) to Level 4 (high level support). Once assigned your package, a dedicated and experienced Rembrandt Home Care Coordinator will work with you to plan the services that you want, the way you want them, in the comfort of your home.

Charter of Aged Care Rights
All people receiving Australian Government funded residential care, home care or other aged care services in the community have rights. Click here to view the Charter of Aged care Rights. These are important rights and at Rembrandt Living we take them very seriously. All staff and volunteers are trained to protect the rights of our people and appropriate systems and checks are put into place. Additionally, our residents, community and allied health care consumers and their representatives have a voice. They can express their ideas, needs or concerns through the Rembrandt Board’s Consumer Reference Group, or through the support of Aged Care Advocacy Group (ARAS).
